How to restore medical documents lost in occupied territory during the war

Because of Russia's military aggression and the constant shelling of towns and villages, many Ukrainians looking for a safe place to live have had to move to other regions of Ukraine or abroad. During relocation and evacuation, it is not always possible to gather all the necessary documents, including medical ones, and many of them may be lost.

Ways to restore lost medical documents:

  • If you need to restore medical records, namely the results of examinations, treatment, inpatient care and surgery, as well as rehabilitation recommendations, contact your family doctor or any other primary care doctor where you are currently staying. The doctor will decide whether to restore the medical documents through the electronic health care system (eHealth) and/or carry out additional examinations so that your treatment can continue.
  • To restore lost documents belonging to a person with a disability, contact the social protection authority, which has access to the Centralized Database of Persons with Disabilities and can issue an extract for the person.
  • If other documents need to be restored, or if you need to renew a certificate from the medical and social expert commission (MSEK) or undergo a medical and social assessment, contact the MSEK where you are currently staying: MSEK doctors, together with doctors of healthcare facilities, will advise on how to resolve the issue.

Importantly, all internally displaced persons are examined by the commissions extraterritorially, that is, where the person is currently staying.

The Ministry of Health of Ukraine reminds you that the deadline for re-examination of persons with disabilities and children with disabilities, if it falls during martial law in Ukraine, is postponed to no later than six months after martial law ends or is lifted.

Today, all medical and social expert commissions in Ukrainian-controlled territory are operating as usual and, together with social protection authorities, are helping all internally displaced persons restore and obtain lost documents.​
