How healthcare facilities plan to spend money received from the National Health Service of UkraineSubvention: UAH 1.8 million. We analyzed which patients were least likely to come to us to sign declarations and found that it was people aged 65 and over. We assumed this was because these people often have no internet access and do not always watch TV. We have now printed a call to choose a doctor, with information on how to do it, on the back of utility bills. It worked: people of this age began coming to us more often.

Subvention: UAH 2.4 million. Under the contract with the NHSU in August: UAH 4,242,260.45. Declarations signed: 47%. Patients served: 133,342.

With the additional funds, we will financially motivate the doctors with whom patients have signed the most declarations and redistribute money in their favor. In June, a family doctor of the highest qualification category earned UAH 8,186 after tax. He has already been chosen by 1,800 patients, and in July and August, when NHSU payments for patient care arrived, this family doctor received UAH 13,730 a month after tax. The nurse who works with him used to earn UAH 5,400 and now earns UAH 6,250. Twelve doctors have already reached the maximum number of declarations. Our first priority is doctor motivation and salaries. We based pay on qualification categories, which allows us to take length of service into account. We have drawn up a table for calculating the bonuses doctors will receive depending on the number of signed declarations.


In August, a doctor who reached the maximum number of declarations (family doctor: 1,800; general practitioner: 2,000; pediatrician: 900) took home UAH 15,700. Before the contract with the NHSU was signed, such a doctor received UAH 5,500. Nurses' salaries were UAH 4,150 and are now UAH 9,000 (this depends on the number of declarations of the doctor the nurse is assigned to).

We also plan to improve our facilities and equipment, and we are already renovating several outpatient clinics. Together with the chief economist, we have started planning the budget, taking into account the funds we will receive from the NHSU.

With the support of the city authorities, we started preparing to sign contracts with the NHSU a year in advance. First, our facilities became autonomous and were computerized. Now 75% of the outpatient clinics have computers, and in August we held a tender to buy the rest of the equipment. Many staff members did not know how to use a computer, but after special training they quickly got up to speed. Following the recommendations of Acting Minister of Health Ulana Suprun, we set up answering machines on the registration desk phones to inform people around the clock and began using modern mobile equipment to sign declarations at patients' homes. A family health day was held to inform the public.

Subvention: UAH 1.1 million. Under the contract with the NHSU in August: UAH 2.021 million. Declarations signed: 67%. Patients served: 51,280. Under the new funding system, we receive 100% more than under the subvention, so the additional money will go partly to salary increases and partly to a reserve fund, and we plan to spend the rest on developing the center's services, facilities and equipment. We plan to buy TV screens to inform patients about each doctor. Later, we plan to renovate the outpatient clinics and create comfortable waiting areas for children and adults.

We revised the staffing schedule in advance: now almost every outpatient clinic has one doctor and one nurse, which made it possible to raise salaries not only for doctors but also for nurses. In June, a family doctor earned a maximum of UAH 6,800; after 1,800 patients signed declarations with him, he received UAH 10,508 in July and UAH 12,930 in August. A nurse earned UAH 3,727 in June, UAH 5,176 in July after NHSU funds arrived, and UAH 6,134 in August. On 31 August 2018, the center approved a new collective agreement. It establishes a new form of pay that will make our medical staff more motivated to provide high-quality medical services. We also plan to introduce indicators to differentiate salaries. They will depend on the number of patients, services provided, complaints, completion of medical records, and so on.

The center already has an urgent care room. In Selydove, three paramedics work there every day from 8:00 to 20:00. If necessary, they call a doctor. From October, the room will be open around the clock. All outpatient clinics have notices with the room's address and opening hours.
